Fast and easy, in your monitored host

UserParameter=mysql.query,echo "select count(*) from hosts"| mysql -N -uroot -p<YOUR_MYSQL_PASS> zabbix

Restart you ZBX-Agent and test from you ZBX-Server:

zabbix_get -s<monitored-HOST> -k mysql.query[]
