Simple check an ip adrress behind the host

i'm newbie in zabbix and i have a question from you dear experts
i have a mikrotik router that i had add as a host and monitor it over SNMP interface with 10.128.0.1 ip address
behind this router is a PC that runs windows 10, with ip address 10.128.0.6.
is there any way to use simple check the windows pc (ping and TCP port 3389) with template ? i figured out that i can do this by add a item and enter host address manually [ex: icmpping(,10.128.0.6,3)]
but i have 160 routers in network with same windows and i want to implement this with a template , but in couldn't find a way to tell the template to add a simple check with ip host ip address + 5
** need to mention that all my windows pcs are have a host ip address +5 , it means if host address is 10.10.10.1 so the windows ip will be 10.10.10.6
