Though zabbix maps leave a lot to be desired, I can imagine a major overhaul would be a massive undertaking.
One minor improvement that seems like it would simple to implement, would be the ability to have multi-sized images. Currently, it's common to have a bunch of the same images at multiple resolutions. This makes finding images complicated, and cluttered.
If each "image" was actually an array of images, the clutter could be reduced and you could still maintain a variety of image sizes.
The icon section of the map dialog would have to be modified to add a size dropdown. And the image dialog would have to be modified to allow loading of multiple image sizes.
If the image sizes were a set of static options, this multi-image setup would be a modest modification.
