Database in the sql server

Currently I need to monitor approximately 500 database in the sql server and I am not able to locate the counter in perfmon to measure the latency of writing and reading by perfmon-> zabbix-> grafana

Can someone help me with the best way?

Whole monitoring doesn't work that way.
First you need to define you monitoring metric depends what you are monitoring (and/or other things) than you will be able to find something in monitoring data.
