Trigger actions only send mail to one user

Trigger description like "Mobile-NG Server failed to send SMS"
Trigger description like "just been restarted"
Trigger value = "PROBLEM"

I'm trying to create an action that sends alerts only when a counter increments and no recovery messages.

Why does only the first user listed receive emails with this action?

Do both users have access to the host referenced in the event?
A recent change fixed / changed action destination permissions, requiring users to actually have (at least RO) permission on a host.
