    - for the cpu load, the trigger should go back to normal state after some time if the cpu load decreases
    - for the Apache server, it may be normal to see the process is not running, because of the different names depending on the distribution you are using (on some distros, it's named httpd, on others apache etc ...)

Zabbix comes with some out of the box features, but you often have to tune them manually in order to get items / templates / triggers in the way you want them to be

Oh and btw, I advise you to upgrade to Zabbix 1.6.4

I upgraded from 1.6.2 to 1.6.3 and then to 1.6.4 doing the following :

        - compile the new sources on the server hosting the Zabbix Core (both Zabbix server & Zabbix agent binaries got replaced by the new one)

        - delete the old frontend php files and replace them by the new frontend files on the server hosting the frontend

No changes are required to the database (in my case at least, but since you would upgrade from 1.4.x to 1.6.x ..... I'm not sure if it's the same. You should ask to someone who knows before doing it )
