monitor single process cpu use and mem use

i want to use zabbix monitor a single process's cpu use and mem use,in my taskmanger,display four same process name,but i donot know how to do,please help me,thanks!

You can do this by using the Performance Counter of that Process like this:
perf_counter[\Process(namgr)\Processor time (%)]
