First of all you need to tell us what OS you are using and what MTA you are running. Based on the error message, I am gonna assume that you are using a linux system with sendmail.

The error message says it all. Your system is not allowing your process - in this case zabbix - to connect to SMTP server. The obvious place to look for it is /etc/hosts.allow. If you are using tcp wrappers, then add this line in /etc/hosts.allow - "sendmail: 127.0.0.1" (without quotes of course). What this line does is lets tcp wrappers know that you are going to allow localhost to connect to smtp. If that is already there then you might be using Selinux and you probably don't have the right policy set up for it. But, I'd say that most probably is your tcp wrappers.

      she's running gentoo.

chances are very good that there is no MTA installed at all.

      try

      emerge -u sendmail
      rc-update add sendmail default
      /etc/init.d/sendmail start
