Help with Zabbix Actions

All,
I have my Zabbix environment set up and triggers are working but I am trying to set up actions for my triggers. I have a group called Remote Hosts. I have a trigger setup to tell me if the host is unreachable by using a ping to the host every minute. I can setup an Action for an individual host and then kill the connection to it and an email will trigger. I would rather not set up an individual Action for every host I have in Zabbix so I have tried a few different conditions to get this to work and I have them listed below but none of them are working. Any help here would be great and much appreciated.

The name of the trigger in Zabbix is "Remote host {HOSTNAME} is unreachable" So in the dashboard it will say "Remote host Tire Dealer 1 is unreachable"

Here are the different Action Triggers i have defined and none work.

Trigger name like "Remote host is unreachable"
Trigger name like "Remote host {HOSTNAME} is unreachable"
Trigger name like "& Remote host & is unreachable &"

Try adding 2 conditions using the AND type of calculation:

(A) and (B)
(A) Trigger name like "Remote host"
(B) Trigger name like "is unreachable"


and I would suggest adding
Maintenance status not in "maintenance"

Another approach could be
(A) and (B) and (C)
(A) Host group = "Remote host"
(B) Trigger name like "is unreachable"
(C) Maintenance status not in "maintenance"
