actions - triggers - changes not applied

does anyone have the similar problem?

we have zabbix 2.2.1 and a bunch of trigger actions for sending email notifications.
problem is, if we make some changes from time to time, it seems it still keeps the old settings.
for example adding/removing users, changing default subject/message.
i've tried disabling/enabling action, cloning, etc
(e.g. - i change the default subject, click save -> it still sends the old subject email)

it is not doing 100% time though.

is it some kind of a bug?

it is still there even after restarting zabbix_server. my email is not in the trigger, but i keep receiving notifications
