trigger action not being triggered

Hi,
I have a trigger action that is not working
although the action is set properly, the trigger won't start the action
I tried creating a new trigger - Didn't help

How can I debug this?

See,
I have this Missing Route alert, which should have triggered the Missing Route action command
However, it is not taking any action (no green 'Done' in the action column)
I noticed that happen in several triggers already which makes the Zabbix monitoring system to be unstable and unreliable.

Your help is appriciated
