Action not working for triggers on web scenarios

We are using the pager duty integration for an action to sent alerts to pager duty. The action has only 1 rule "Trigger severity is greater than or equals High". In our prod environment all of our triggers are set to High. I can see the triggers for our web scenarios are triggering, but there's no action happening? This does work for all of our other triggers that are associated with regular Items and not web scenarios? Any ideas? I did turn on the debug logging, however I wasn't sure what I was looking for in there to help us out. Please let me know if you need any additional information.

We found the issue. The hosts that had this web scenario applied didn't have permissions on the pager duty service group. Thanks for looking.
