Actions not fired

Hi,
I noticed that a working action suddenly stopped working and when the trigger is alerting, the action isn't fired.
This is not the first time this is happening, How can I debug this? Any Workaround?

Zabbix Server: Zabbix 4.0.2 + MySQL

Since the action isn't fired, it doesn't appear in the action log.

I ran the following query to check for escalation, and i saw that some events have escalations.nextcheck with the date 1970-01-01 any idea why is this happening?

select events.eventid, escalations.triggerid, FROM_UNIXTIME(events.clock), FROM_UNIXTIME(escalations.nextcheck) from escalations as escalations JOIN events as events ON events.eventid = escalations.eventid order by FROM_UNIXTIME(events.clock) DESC;

Since the action isn't fired, it doesn't appear in the action log.

Maybe try to create a new action (something unrelated to this action) and test it. see if it works.
If it does then ill suggest recreate the action, it could help - maybe it is some kind of zabbix 4 bug?
