Zabbix monitoring 2 postgres clusters on 1 linux server with template pg_monz

All,

I am pretty new to zabbix.
I want to monitor my 2 postgres clusters (Version 11.9 , port 5432 and 5433) on 1 server (Red Hat Enterprise Linux Server release 7.8) with the pg_monz template.
The zabbix version is 4.0
I can not find any installation/documents how to do that.

Any suggestions?
