Confused About Notifications

I'm a little confused about when Zabbix sends notifications to users defined in the admin backend.

Is the only time notifications are sent out when they are defined under an action?

For the most part, yes. One exception to the rule is the "Database down" message. That one is defined under Administration --> General --> (top right dropdown) "Other", then "Usergroup for database down message" is generally set to Zabbix admins, I believe. You can set that to whatever group you want.
