you can use dns-name instead of the IP. But that require a working DNS-Server

Or you can use active items - https://www.zabbix.com/documentation.../activepassive

So do i have to make changes in the zabbix server or in the agent conf file of the hosts?

I also believe that you could use the zabbix-agent and have that point to your Zabbix server, that way whenever there is a change of IP it would change on the server end.
