Count problems of each host by an item

I need to get the problems for each host I created. I put triggers in all of them and now I need to get all the problems that they return, but I want to do an item that returns to me the number of problems in their respectively host.
I was trying to do this by a query on zabbix DB, but unsuccessfully cause the table "problems" seemed confusing to me... but before I focus on this way:
It is possible to do what I want? What is the best way to do this?
