Locked out of server with "Cannot Bind LDAP"

I am locked out of our Zabbix server. I changed the ldap settings and tested using the Test button. After a successful test now no one can log into the server. My AD acct fails with "Cannot bind LDAP server, and so does the local Admin acct.

How can I get back into the GUI when ldap fails?

5.0, centos, mariadb, apache.

Try to do the following to disable LDAP:
ssh to server -> open MySQL -> use zabbix; -> show tables; -> authentication is in 'config'

update config set authentication type = 0
