How to Alert on Database State? MSSQL

I want to create a Trigger based on the discovery data in the Value Mapping section as seen in the attached screenshot. From what I gather these value mappings are basically just human readable translations or clarifications? However, I don't see any data collectors that relate to DB State and Service State any where else? No items, no triggers collect this data that I can find.
Mainly, I want to trigger if a MSSQL DB state = 4 and Service state = 0
I couldn't find any triggers or items that base their data on the Value Mappings to clone so I'm not really sure how to get started?

Search for "state" you should find all those items and triggers that gather and report data about DB state....

So I've been trying to create an item based on the collected items in that link. I've attached screenshots of the create item screens and the following error I get when I click add. Basically it says there is already an item that uses that key. However, if I search for items by key nothing is returned.
