Cry for help on Item prototypes

I am encountering a problem with the discovery rule of a custom template I have created in Zabbix. Although I am able to retrieve data for the items, the Item prototypes are not functioning properly. I have confirmed that the OIDs and keys are correct, and there are no errors in the front end or back end.

To troubleshoot the issue, I have traced the discovery logs, but unfortunately, I have not been able to find any helpful information. I am attaching screenshots of my template and the latest data for reference.

My question is: What could be the possible reasons for this behavior? Can someone provide guidance on how to troubleshoot this issue and get the Item prototypes to work properly? This is the first time I am facing this issue and its eating me alive.

Any assistance or insights would be greatly appreciated.

    Thank you.
