      Install Zabbix agent

You'll need to install Zabbix agent on the remote MySQL server, configure the Zabbix agent on that remote machine and open ports 10050,10051 to the Zabbix server.

After that, in your Zabbix server, add your remote MySQL and in the template you can choose the "Template App MySQL"

      Good Luck
