No Graph Data

Hi all. Here's specs of the setup first:
OS: CentOS 7.2.1511
Zabbix Version: 3.0.4

I have a combination of CentOS 5 and 6 clients that have the agent installed as per the instructions from Zabbix. The guide I've been following is here:

I've added the item within the server's web interface and see no errors from the logs on the various agent machines. Still not seeing any data. Here's the latest excerpt from one of the CentOS agent machines (which is the same across the rest):
