Long running Zabbix server reduced to "Login name or password is incorrect."

Our Zabbix server has been set up since July 6, 2016 and has always been running, but recently I noticed that the Zabbix server was shutdown (agent still running).

I was unable to find any logs regarding the issue and start up seems fine when I restart the server. Whenever I try to login on the GUI, the page seems to spin for about 5 minutes before ending up with an error: "Login name or password is incorrect.", although I know the password is correct (same goes for the other users, also running into incorrect password).

I also tried changing my password from the MYSQL database, but it doesn't seem to do anything. The error continues to pop up. I can connect to my AWS RDS MYSQL database from the instance just fine, I see the database and users all present.

OS: Ubuntu 14.04
Zabbix Version: 3.0.3

I did not check the Zabbix server very often, so I was unaware of when the server actually failed. If there is any help I would appreciate it.
