custom email for multiple hosts

is it possible to query all hosts with specific tag and generate and email with the following schema:

host	status
server1	OK
server2	FAIL - error description
server3	OK

[Sebastian]

There would be nice to have also column "Last OK date"
to have possibility to export all hosts and quickly find disconnected devices
