New Install, Admin acount does not work, login screen missing menu?

I just installed Zabbix. everything seems to have went fine. I go to the url, and am presented with a login screen. I tried the "Admin" account with no password, however this did not work. I tried system accounts, they also did not work.

I think I'm missing a menu up top above the login screen.. the very top just shows:

[ERROR:Login name or password is incorrect]

I do not see a link to "CONFIG" as the documentation states...


why might the menu be missing?

System is FreeBSD 5.2-RELEASE with Server: Apache/1.3.29 (Unix) PHP/4.3.4

the data.sql file was not properly imported! Fixed.
