        I had same issue.
        Correct permissions/ownership to:
        -rwsr-sr-x 1 root zabbix 31400 Jan 31 2013 /usr/sbin/fping
        -rwsr-sr-x 1 root zabbix 31408 Jan 31 2013 /usr/sbin/fping6

Make sure it is not a SELinux issue.

Yes, it was described in manual

              5.3 ICMP pings

Zabbix uses external utility fping for processing of ICMP pings.

The utility is not part of Zabbix distribution and has to be additionally installed. If the utility is missing, has wrong permissions or its location does not match the location set in the Zabbix server configuration file ('FpingLocation' parameter), ICMP pings (icmpping, icmppingloss, icmppingsec) will not be processed.

fping must be executable by the user Zabbix daemons run as and setuid root. Run these commands as user root in order to set up correct permissions:

              Code:
              shell> chown root:zabbix /usr/sbin/fping
              shell> chmod 4710 /usr/sbin/fping
              Sincerely yours,
              Aleksey

                Hi,

in my case, SELinux (CentOS 6.5) was blocking fping after setting the right permissions. This was the solution: https://support.zabbix.com/browse/ZBX-760

Also, icmpping can only run from the Zabbix Server, not from agents in other machines. I think the ability to create an item in a host that doesn't have the Zabbix Server template assigned is a bug...

                hope it helps.
