Host suddenly unreachable

Hi. I'm running Zabbix 1.0 on a rhel3 box, with net-snmp v5.0.9-2.30E (redhat's release). I'm monitoring a cisco 7513 via SNMP. I rebooted the 7513 a few days ago, and ever since then Zabbix is stating that it is unreachable. I've verified connectivity with snmpget and snmpwalk at the cli on that box, with the same information that Zabbix is polling, and it works fine. I tried clearing the status in the db, and it goes right back to unreachable. In the log file all i see is this:

so i've played a bit more... i tried adding the device in again, and even utilizing different IPs on that device, still no bueno.

Is there any way to troubleshoot how zabbix is trying to connect to the device? Coincidentally this is device is the core of my network so its kinda important that I can monitor it.
