Zabbix, new action, error when create

Hello.

Zabbix 4.2.1 . When I want to create Action, I get this screen (Action new.jpg attachment).

And, when I want add something, after few seconds, I get this (error.jpg attachment).

What is that and how can I fix it?

Zabbix 4.2.1 + nginx + php-fpm (php ver. 7.2).

In Nginx log there is:
