LDAP/AD Authentication Setup seems to fail silently

I'm setting up Zabbix user authentication against our AD domain.

created a zabbix user with my AD username
escalated the user's privileges to Zabbix Super Admin
logged in as my AD username
filled in all the values in the LDAP form with info from our AD admin and my AD username and password
Hit test got a blank page
Tried to update the AD entry page before hitting test, also got a blank page.
Used the dynamic utility to increase server logging level, re-entered. I see nothing relevant in the logs.

I'm looking for logs or other places I can find error messages that will help me resolve the problem.

    TIA
    Dave

    More troubleshooting on this issue.

I've verified that the ldap server I'm entering can be resolved by name.
I've used netcat on the zabbix server to verify that I can reach port 389 on the ldap server.

On my latest attempt, l left the browser open for 45 minutes after I pushed "Test" on the ldap config page. No error was returned and the page never redrew.

I'm still looking for logs or other debugging info that I could use to help get LDAP/AD authentication going. Know of any?

    TIA
    Dave
