Actions not working for certain groups?

I recently noticed that one of the 3 cluster servers is not sending out notification, these servers have everything configured the same (templates, triggers, etc) except for one thing, server that is not sending out notifications is not part of two groups Group 1 and Group 2, it's only part of Group 2. As soon as I place it into both groups it starts sending notifications. here comes the interesting part... Action doesn't have Host Group condition, it should cover all of them in theory, or am I missing something?

Looking for a best way to understand/troubleshoot why this is not working.

Running latest 5.4.3, but this issue has been present for at least a year and we update servers monthly.

That was the problem. He wasn't, corrected that now - instead of specific groups he has access to all the groups as this is Zabbix account is used to send to distribution group.

Is the user, to whom you are sending message (that blacked out part of picture), able to see group2?...
