How do I convert "MySQL by Zabbix agent" to work with an active agen

I converted all of my passive agents over to active agents because they are on a protected network and the zabbix server does not have access to them. This has worked and the active agents are phoning home and I'm getting data.

Except for "MySQL by Zabbix agent". What do I need to change to make that template work with an active agent?

You can change the type of the item whose type is "Zabbix agent" to "Zabbix agent(active)". If that doesn't work, you can see that some error message is outputted. Please tell me the error message and the contents output to the log.
