Verified template item using zabbix_agent but still marked as item key not supported

I could verify that a custom template item using zabbix agent. However, for hosts that use this template, the item is still marked as Not supported (Unsupported item key).

Any suggestions on how to debug this further. Both agent and server log files don't have any useful info.
