Some Triggers never "untrigger"

When this trigger goes off, it never goes to "OK" state even if the zabbix graphs/latest date show as it no longer being in a problem state. This happens to a few other triggers too (the default entropy one also)

I am also having a problem with the database where very frequently the database just hangs/crashes and I have to kill and restart everything (I have a script that checks this now). I am wondering if the server is down when the trigger returns to a non-problem state but the server is down maybe zabbix is missing it or something?

This is a zabbix install from maybe 1.8 upgraded till 3.0

According to the trigger expression, if minimal value for 90 minutes is less than 99, status is OK, but if > then PROBLEM
Can you show picture from Item configuration and Latest data where supposed to be OK state.
