Zabbix agent on X is unreachable for 5 minutes flapping

I have a Zabbix Server which monitor around 250Host.
In those host only few host (around 10 host) flapping between OK and Problem state on "Zabbix agent on X is unreachable for 5 minutes" trigger. Not immediately flapping between two states it takes some time(around 2-5mins) before changing state.

Can anyone help me?

Can you please troubleshoot the following things:
1. Ping for 2-5 minutes, see if the connection is stable between the Zabbix server and agent?
2. What are the Timeout setting values for both Zabbix server and agents?
3. Check the internal Zabbix Server processes and see they aren't loaded fully (Monitoring --> Graphs --> <Zabbix server>).
