Why is my trigger action not triggering for only one host group?

I have a trigger action that is supposed to trigger on warning or higher for two different host groups. The trigger alert works for one host group, but not for the other host group. Zabbix server version is 3.4.2.

The servers in the non-working host group are in two host groups. A second trigger action on the second host group works fine. The action log doesn't show that the action triggered for the one trigger action, but it did for the other.

I've removed and recreated the host group multiple times plus recreated it with a different host group name to no avail. I even reduced the host group to have just one server in it.

Ideas? The attached trigger action fires for the DB Servers host group, but not for the MS SQL Servers host group.

I upgraded the Zabbix server software to 3.4.6, but the problem still remains.

I'm in very similar problem. one node is not sending any emails. There are triggers created in the front end but no alerts!!

Does your action have the correct setting for the severity of the trigger?
