We just use the built in Zabbix templates for Zabbix Server and Zabbix Server for the application along with traditional Linux platform monitoring.

Do these approaches not work for you, are you looking for something for specific?

I wouldn't use external checks, they are limited to running in the timeout value of your server or proxy. I would use scripts scheduled by cron to send data with zabbix_sender.

Some things over time that I have added (some may not be needed on current releases):

Size of proxy config (from proxy.log)
events by type per hour (flooded by internal events)
