Zabbix MySQL Configuration Issue - create.sql.gz failing

I'm following the instructions here to install Zabbix fresh on Centos7 with MySQL and it's failing at the d. Create initial database step.

I upgraded 4.0 LTS to 4.2 on CentOS7.

And, When I accessed GUI by brower, "Configuration" & "Administration" menu does not display on GUI.

My upgrade flow was follows.

I'm trying to setup Zabbix on CentOS 7 (inside a Virtual Machine). After installation, when I try to start zabbix-server (systemctl start zabbix-server.service), I receve this message:

Job for zabbix-server.service failed. See ‘systemctl status zabbix-server.service’
