Discovery of single IP with multiple ports

I'm trying to configure discovery rules on Zabbix 6.0, where we have some network switches that we are using port forwards for,
ie we have a single IP visible but each port points to a different device

I can get my SNMP discovery rule working, but my problem is that it will only add a single host and then multiple SNMP interfaces, 1 for each port
That wont work for me as I need each port monitored independantly to each other.

I'm using the SNMPv2-MIB::sysName.0 SNMP agent chekc so each port that responds will give a unique response and have the device uniqueness set to the SNMP check.
Is there a way I can get the discovery action to create a seperate hsot based on the response of the SNMP check?
