Zabbix Actions Triggers losing configuration

Hi there,

I am having the following problem on my Zabbix 3.4.12:

The configurations from the Configuration->Actions->Event source>Triggers are mysteriously disappearing after a while, sometimes takes couple months to disappear, sometimes couple days.
I noticed that the the Reports->Action Log also lose all the history when this happens.

Can someone help me?
