Incorrect graph headers after upgrade from 5.0.30 to 6.0.12

After upgrade from 5.0.30 to 6.0.12 some graph headers look like Zabbix shows variables instead of their values

localhost CPU $2 time
localhost free disk space on $1
localhost Zabbix $4 $2 processes, in %

Has anyone had a problem like this?

You need to replace the positional macro "$N" with the "Nth" field in the key in the template, for example:

system.cpu.util[,idle]: "CPU $2 time" -> "CPU idle time"

Do you happen to know whether it is possible to change all problem macroes at a time. I mean, is there any script to do that?
If I upgrade Zabbix from 5.0.30 to 5.4.X and then to 6.0.12, will that help?

No, no upgrade will change your item names. You need to change your templates after upgrade. I suppose those are all in prototypes, so one littel chaneg will affect many hosts..
