Unable to identify host on zabbix server. Setup (host--> proxy --> server)

I am trying to install zabbix 3.4.4. I have successfully installed zabbix agent on a host. Zabbix proxy on another host and Zabbix server on another host. When I connected zabbix agent directly with zabbix server. The Zabbix server was able to identify the host, but when I try to connect it via the proxy the server is unable to identify the host.
From Zabbix Server (Admin --> Proxy) I can see that the server is reaching the proxy as last seen is always 1 second or 0 second. Proxy has been configured as Active. (ProxyMode=0).
Any ideas what can be setup wrong ?
